To help us insure the prompt exchange of products that qualify for replacement under warranty, (or for factory repair of non-warranty items), please fill in all information requested on this form, and then click Send to email the form to Optica Technologies. We will promptly provide you with an RMA number, which is your authorization to return the product. You will also receive complete return shipping instructions by email. For warranty items, we will make commercially reasonable efforts to ship a replacement product not later than two (2) days after we receive your defective product, in accordance with the terms of our Limited 1-Year Warranty. For factory repairs, we will communicate a time and cost estimate before proceeding with the repair work.
